2. Proof of Residency

Submit one full-page document from the list below.
The document must clearly show your name, address, and a valid date within the required timeframe.

Housing Documentation

• Home mortgage, lease, or loan contract (within 6 months)
• Current homeowner’s or renter’s insurance policy (current year)

Utility or Insurance Bills

• Utility bill (gas, electric, phone, cable, or heating oil) – no older than 60 days
• Car insurance policy or bill – no older than 60 days

Vehicle and Registration

• Massachusetts motor vehicle registration (current)

Government-Issued Documents

• First-class mail from a federal or state agency – within 60 days
• Property or excise tax bill (current year)
• Census or Census Verification (current year)
• Jury Duty Summons (within 3 months)
• Voter registration certified by city/town clerk

Educational and Professional Documents

• Tuition bill addressed to you – within 6 months
• Letter from a Massachusetts school official confirming residency (within 1 month)
• Massachusetts-issued professional license (current)

Other Accepted Documents

• U.S. Marriage Certificate (within the past 6 months, includes address)
