CED Clinic (“CED Clinic,” “we,” “us,” or “our”) operates cedclinic.com (the “Site”). This policy explains what information we collect through the Site, how we use it, and the choices you have. It covers our website and the forms, scheduling, and payment tools on it, it does not replace or substitute for any separate notice we provide about the medical information collected as part of your clinical care.
Information We Collect
Information you give us directly
- Contact and intake forms. When you fill out a form on our Site (for example, to request a consultation, ask a question, or reach our clinical or business teams), we collect what you enter, typically your name, email address, phone number, and the details of your message. Our forms are built and hosted using JotForm; JotForm’s own privacy policy governs how it handles data in transit and storage on its platform.
- Appointment scheduling and payment. If you book or pay for an appointment through the Site, that transaction is processed by Square. We do not store your full payment card details ourselves, Square handles that in accordance with its own security standards and privacy policy.
- Purchases. If you make a purchase through any shop or product pages on the Site, we collect the information needed to complete that order (such as name, shipping address, and payment details processed through our payment provider).
- Comments. If you leave a comment on the Site, we collect the content of the comment along with your IP address and browser user agent string, standard practice for spam detection on WordPress sites.
Information collected automatically
- Analytics. We use Google Analytics and Google Tag Manager to understand how visitors use the Site, pages viewed, general location (city/region level), device and browser type, and referral source. Google’s own privacy policy governs its handling of this data.
- Jetpack. Our Site uses the Jetpack plugin for performance, security, and statistics, which may collect similar technical and usage data. See Automattic’s privacy policy for details on Jetpack’s data handling.
- Cookies. The Site uses cookies for basic functionality (like remembering your comment details) and for the analytics tools above. Most browsers let you block or delete cookies; doing so may affect how some parts of the Site work.
How We Use Your Information
- Responding to your questions and consultation requests
- Scheduling, confirming, and managing appointments
- Processing payments for appointments or purchases
- Sending you information you’ve requested, such as appointment reminders or, if you’ve opted in, newsletters and updates
- Understanding and improving how visitors use our Site
- Maintaining the security of our Site and detecting spam or abuse
We do not sell your personal information.
We share information with the service providers named above (JotForm, Square, Google, Jetpack/Automattic) to the extent needed for them to provide their services to us. We may also disclose information if required by law, or to protect the rights, safety, or property of CED Clinic, our patients, or others.
How Long We Keep Information
We retain contact form submissions and appointment records for as long as reasonably needed to respond to your inquiry, provide the service requested, and meet our recordkeeping and legal obligations. Comment data is retained indefinitely so we can recognize follow-up comments. You can ask us to delete information we hold about you, subject to any legal or clinical recordkeeping requirements that may limit that.
Your Choices and Rights
- You can decline to submit a form on our Site, though this may limit our ability to respond to your request.
- You can opt out of Google Analytics tracking using tools like Google’s Analytics Opt-out Browser Add-on.
- You can unsubscribe from any newsletter or marketing email using the link in that email.
- You can ask us what personal information we hold about you, and request that we correct or delete it, by contacting us below.
Children’s Privacy
The Site itself is intended for adult visitors. CED Clinic does provide pediatric care as part of its clinical services; information about a minor patient collected in that clinical context is handled under our clinical policies and applicable law, not through unsupervised use of this website by a child.
Changes to This Policy
We may update this policy from time to time. We’ll change the effective date above when we do. We encourage you to review this page periodically.
