The choice between these agents fundamentally alters treatment success rates and requires evidence-based patient stratification to maximize therapeutic benefit within the obesity-diabetes spectrum.

Liraglutide and tirzepatide represent two distinct pharmacologic approaches to weight management and glycemic control in adults with obesity and type 2 diabetes. Liraglutide is a glucagon-like peptide-1 receptor agonist that functions as a GLP-1 receptor agonist, while tirzepatide is a dual GIP/GLP-1 receptor agonist that activates both the glucose-dependent insulinotropic polypeptide and glucagon-like peptide-1 pathways.
