Perioperative factors predict opioid refill patterns in the critical 90-day window after head and neck surgery, identifying modifiable risk factors for persistent opioid use.

What This Study Teaches Us

This study identifies specific perioperative factors that predict opioid refill patterns during the critical 90-day post-surgical window when acute pain transitions to persistent opioid use. The analysis across three distinct time periods provides insight into how risk factors may change over the recovery trajectory.

Why This Matters

Head and neck surgery patients face unique pain management challenges due to procedure complexity and anatomical considerations. Understanding which patients are at highest risk for requiring refills helps clinicians proactively adjust pain management strategies and implement targeted interventions to prevent persistent opioid use.

Study Snapshot
Study Type Retrospective Cohort Study
Population 4,132 adult patients undergoing inpatient otolaryngology-head and neck surgery procedures, median age 62 years, 45.3% female
Intervention Inpatient OHNS procedures with minimum 1-day hospital stay
Comparator Analysis of refill patterns across different time periods and patient characteristics
Primary Outcome Opioid refills at 1-30 days, 31-60 days, and 61-90 days post-discharge
Key Finding Identification of independent perioperative risk factors associated with opioid refills during critical transition period
Journal JAMA Otolaryngology-Head & Neck Surgery
Year 2024
Clinical Bottom Line

Perioperative risk stratification can identify head and neck surgery patients most likely to require opioid refills during the critical 90-day transition period. This enables targeted pain management approaches and closer monitoring for patients at highest risk of developing persistent opioid use patterns.

What This Paper Does Not Show

The abstract does not specify which particular risk factors were identified, the magnitude of their associations, or whether interventions targeting these factors actually reduce persistent opioid use. The study also cannot establish causality between identified factors and refill patterns.

Where This Paper Deserves Skepticism

Single-center data may limit generalizability across different practice settings and patient populations. The study design cannot account for patients who obtained opioid refills from providers outside the academic medical center, potentially underestimating true refill rates.

Dr. Caplan's Take
I see promise in risk stratification tools for post-surgical opioid management, but the clinical utility depends entirely on which specific factors were identified and their practical applicability. Without knowing the actual risk factors, I cannot assess whether this changes my approach to prescribing or monitoring in head and neck surgery patients.
What a Careful Reader Should Take Away

This study advances our understanding of opioid refill patterns after head and neck surgery by identifying perioperative predictors during the critical 90-day transition period. However, the clinical actionability awaits full publication details on specific risk factors and their relative importance.

FAQ

What is the critical time window for monitoring opioid refills after head and neck surgery?

The 90-day period after discharge represents a critical window for the transition from acute to persistent opioid use following otolaryngology-head and neck surgery. This study specifically examined refill patterns at 1-30, 31-60, and 61-90 days post-discharge to identify when patients are most at risk for developing persistent opioid use.

Which patients are at highest risk for requiring opioid refills after head and neck surgery?

The study identified specific perioperative risk factors associated with opioid refills, though the complete list is not provided in this summary. Risk assessment should consider both opioid-naive and opioid-experienced patients, as the study included adults regardless of prior opioid exposure status.

How has opioid prescribing changed in head and neck surgery over recent years?

From 2017 to 2023, there was a substantial reduction in total oral morphine equivalent prescribing for head and neck surgery patients. Despite this reduction in initial prescribing, understanding refill patterns remains crucial for preventing the transition to persistent opioid use.

Should opioid refill protocols differ based on time since surgery?

Yes, the study’s division of the 90-day period into three distinct intervals (1-30, 31-60, and 61-90 days) suggests that refill risk factors may vary by timeframe. This temporal analysis can help clinicians develop time-specific protocols for evaluating and managing opioid refill requests.

What clinical implications does this research have for post-operative pain management?

This research provides evidence-based guidance for managing opioid refill decisions during the critical 90-day post-operative period when patients may transition from appropriate acute pain management to problematic persistent use. The identification of modifiable perioperative risk factors can help clinicians implement targeted interventions to reduce unnecessary opioid exposure while ensuring adequate pain control.
