> **Clinical Summary:** This review looked at many earlier studies to see whether using CBD products or hemp oil can cause a person to fail a drug test for cannabis.

Background: Hemp-derived products, especially CBD oil and hemp oil, have shown a notable increase in use. Concerns exist about unintended cannabinoid findings (THC, 11-OH-THC, THC-COOH) on drug tests in workplace, road safety, and anti-doping settings, since many ‘THC-free’ products may contain trace THC and oversight of CBD oils is heterogeneous. Methods: Systematic review following PRISMA guidelines of studies published in English from March 2014 to March 2024, searched in PubMed, WOS and SCOPUS. Results: Of 477 records, 12 studies met inclusion criteria. Eight evaluated whether CBD or hemp oil use yields positive cannabinoid findings in biological materials; five reported no results above the limit of detection or cut-off, though trace or near-limit levels were seen. Four studies evaluated conversion of CBD to THC in human metabolism; two of four found no biotransformation or extraction-induced transformation. Conclusions: Positive results arise mainly from product content (undeclared or excessive THC, mislabeling, contamination), matrix, administration route, and analytical sensitivity/specificity. CBD conversion to THC during manufacturing or in the body may pose legal risks in zero-tolerance jurisdictions.
